Appointment Information
We look forward to seeing you in our clinic.
When you come for your first appointment, you will need to bring:
- A referral from your GP or other medical specialist
- Your Medicare card
- Health fund membership card
- Your x-rays and any other scans
- List of medications (if appropriate)
- Your list of questions for your surgeon
You may also wish to complete your Patient Registration Form prior to your appointment.
Completed PDF forms can be emailed to the clinic or brought with you on the day.

The ground floor clinic is fully accessible.
